INTRODUCTION

CNS type 1 is an inherited disease in which bilirubin cannot be broken down causing hyperbilirubinemia, which is a toxic condition. Bilirubin is a substance which is produced on breaking down of red blood cells and further converted from unconjugated bilirubin (toxic form) into conjugated bilirubin (non toxic form). CAUSES

In CNS (Type 1) the reason behind not converting unconjugated bilirubin into conjugated bilirubin is an absence of enzyme that catalyzes the chemical reaction. Bilirubin uridine diphosphate glucuronosyltransferase (bilirubin-UGT) enzyme is the one responsible for removal of bilirubin from the body is found in the liver cells normally but in case of CNS (Type 1) it is absent because of mutation in the gene that provides instruction for its production. It is an autosomal recessive disorder which means mutation is found in both the copies of the gene.

SIGNS AND SYMPTOMS

Signs and symptoms may differ from one person to another also not all the symptoms listed below might be found in one person. Signs and symptoms are caused due to damage to brain and nerve tissues.

• Babies are extremely lethargic and tired
• Weak muscle tone
• Arching of babies back

Conventional treatment of CNS (Type 1) includes 10-12 hours per day sessions of phototherapy to maintain the level of unconjugated bilirubin below the toxic level. Bilirubin chelators like cholestyramine and calcium salts can be used.
