The causes for this condition are not clear according to modern science. There is a rough theory that viral or bacterial infections might have triggered the situation.
If understood from the point of view of Ayurveda then the condition appears after the accumulation of ama dosha in the body. Ama dosha accumulation is the result of intake of vidahi, virudh anna along with a sedentary lifestyle. This ama dosha then travels to various srotas of the body under the influence of vata dosha. This hence results in blockage of srotas and then this sama sannipataja jawra occurs.

Phasic Description
Monophasic Pattern
Patients with monophasic generally have a single episode of symptoms. This typically lasts weeks to months. But it lasts less than a year.
Polyphasic Pattern
In this phasic pattern the patient generally has more than one episode of symptoms. Affected individuals lead a healthy life in between the episodes. This can be for weeks to years. Generally, the episodes tend to be less severe and shorter in duration compared to the initial one.
Chronic Pattern
These patients have persisting symptoms over time.
The combination of symptoms are generally associated with systemic inflammatory disease. This includes:
- Spiking fever of greater than 102 degree fahrenheit. This can occur twice daily.
- A skin rash
- Pain in muscles (myalgia)
- Pain in joints (arthralgia)
- Inflammation

Diagnosis of Adult onset of Still’s disease
The Adult onset still’s disease has following diagnostic criteria:
This criteria is called the Yamaguchi criteria. This is subdivided into minor and major criteria. The major criteria includes fever of at least 39 degree celsius for one week along with arthralgia or arthritis for at least two weeks. The non pruritic salmon cooled rash on the trunk/extremities. The last one is the granulocytic leukocytes which should be 10,000/mL or more.
The minor criteria includes sore throat, splenomegaly, lymphadenomegaly, increased serum aminotransferase or lactate dehydrogenase levels and negative IgM rheumatoid factor and antinuclear antibodies.
Treatment of Adult onset of Still’s disease
The Treatment for this condition includes the following:
- As it is an autoimmune condition then mostly steroids are recommended at first. These do help in reduction of inflammation but can enhance the risk of osteoporosis.
- Conventional DMARDs are also recommended in this case along with glucocorticoid spray therapy in case of chronic condition management.
- The non steroidal antiinflammatory drugs are also given in this case to reduce inflammation and pain in this case. But their long term usage can hamper digestion.
- Biologic response modifiers are also recommended in this case.

- kshane sheeta (hypothermia)
- kshane daaha (fever)
- asthi sandhi ruja (arthralgia, myalgia)
- shiro ruja (headache)
- karna ruja (otalgia)
- tandra (fatigue)
- srotasaam paaka (inflammation of various channels of the body)
- nidra naasha (sleeplessness) and many others.
That’s how the condition is correlated but here also there is one another factor that this samasannipataj jawara in this case is vata dominant along with the involvement of other dosha as well.
Now coming to the management of this condition according to ayurveda. Then
Ayurveda suggests that firstly elimination of ama dosha is to be then shanti of vata prakopa will be automatically seen.
