CHILD TURCOTTE PUGH SCORING SYSTEM FOR LIVER CIRRHOSIS

ABSTRACT

Child Turcotte Pugh scores are used for predicting disease severity and mortality among the patients with the end stage of liver disease. The data regarding usefulness in predicting the short-term outcome of end-stage liver disease in children are not available in India. The aetiology of the end stage of liver disease and the clinical presentation were determined. It is the best universal scoring system for liver failure in the patients with liver cirrhosis so let us discuss the disease for how this method gets used.

INTRODUCTION

There are many clinical conditions which lead to child liver cirrhosis. In Most of the cases of this disease the sign and symptoms are caused by similar etiologies. The infants with liver cirrhosis are mostly caused by biliary atresia and genetic metabolic diseases most of the children in older age tend to result from the autoimmune hepatitis and wilson’s disease The signs and symptoms of cirrhosis in children and adolescents are similar to the signs and symptoms of adults. But in children’s the sign of cirrhosis is very poor weight gain. Complications seen in childrens are very similar to those observed in the adult patients and includes gastrointestinal bleeding caused by gastroesophageal varices, ascites and spontaneous bacterial peritonitis. In patients the attention is paid to the nutritional alterations which are caused by cirrhosis. Childrens are at higher risk for other many nutritional deficiencies.

After the surgical procedure patients mortality rate of class A is 10-20% and the patient mortality rate of class B is 30%. The class A patients are safe candidates for performing surgery. class B patients proceed with the surgery after the medical optimization but still they have an increased risk. surgery is contraindicated in class C patients. The Child Pugh scoring system is designed to predict mortality in liver cirrhosis patients . especially conceptualized by Child and Turcotte in 1964 to guide the selective patients who benefit from surgery for portal decompression, it broke down patients into three categories: A – good hepatic function type , B – moderately impaired hepatic function type , and C – advanced hepatic dysfunction type . The scoring system is used for the five clinical , laboratory criterias to categorize patients serum bilirubin, serum albumin, ascites, neurological disorder, and other clinical nutrition status. The scoring system was first discovered by Pugh et al., substituting prothrombin time for the judgement of clinical nutrition status.
