Abstract
Abdominal Cocoon Syndrome is a rare condition of bowel obstruction. It can occur in males and females but most commonly occurs among young adolescent girls of tropical and subtropical regions. The syndrome is not yet clearly known but is assumed to be due to peritoneal irritation. Introduction
Abdominal Cocoon Syndrome is a condition in which there is complete or partial encapsulation of a small bowel with a fibrous-collagenous membrane with a local inflammatory infiltrate that causes bowel obstruction that can be acute or chronic. Abdominal Cocoon is one of the causes of intestinal obstruction. This condition is different from peritoneal encapsulation in which the whole of the small bowel is encased in a thin accessory membrane. While the symptoms of both conditions can be the same, it is necessary to have a differential diagnosis and rule out the exact disorder. Correct diagnosis and early treatment can reduce future complications and mortality rates. Abdominal Cocoon Syndrome can be primary or secondary.
Ayurvedic Aspect
According to Ayurveda texts the Abdominal Cocoon Syndrome can be assumed to occur due to–
- Unhealthy food habits and wrong lifestyle along with poor digestion power (Agnimandh)
- Weak nervous system
The miscommunication between the brain and digestive tract can cause an imbalance of doshas that can lead to dushti of strostas/ dhatus and these disturbances in the Gastrointestinal tract leads to many disorders including Abdominal Cocoon Syndrome
- Stress
- Depression

Causes
Abdominal Cocoon Syndrome is caused by the thick fibrous collagenous membrane.
- The primary syndrome occurs in adolescent girls with idiopathic etiology (unknown cause).
Whereas secondary syndrome is linked with
- Abdominal tuberculosis
- Peritoneal dialysis
- Liver cirrhosis
- Intra-abdominal foreign body sensation
- Liver transplantation
- Systemic lupus erythematosus and many other causes.
These factors can lead to peritonitis, and the exudation of protein causes the formation of the fibrous sac (encapsulation) that forms Abdominal Cocoon Syndrome.
Symptoms seen in person with Abdominal Cocoon Syndrome are-
- Abdominal pain (several attacks)
- An abdominal mass
- Nausea with or without vomiting
- Recurrent small bowel obstruction
Diagnosis
Diagnosis is confirmed with the following diagnostic methods-
- Abdominal X-ray
- Ultrasonography
- Complete blood count
- CT scan
- CECT of abdomen
- Barium study
Treatment
- Certain medications are prescribed for conservative treatment.
- Surgical treatment- Commonly performed procedures are Laparotomy, etc.
