Abstract

Psoriasis is a non contagious, chronic  disease. Psoriasis is an autoimmune disease in which our immune system becomes overactive, causing skin cells to multiply too quickly. People with psoriasis may experience other health conditions like skin cancer, insulin resistance etc. It occurs in both sexes at any age. The age of onset and severity of disease are often similar in familial cases which means if one parent has psoriasis, the chances of child being affected is 10% – 15%.

Introduction

Psoriasis is a chronic inflammatory skin disease. It is an autoimmune disorder. Psoriasis is characterised by erythematous scaly plaques that mostly affect the trunk, scalp, nails, limbs especially on the elbow and knee. This occurs due to overactive immune system which speeds up the skin cell growth and causes inflammation of the skin. In ayurveda psoriasis is characterised in Kushta disease (skin disease). Kushta has two types: Kshudra kushta (minor skin disease) and Maha kushta(major skin disease). Psoriasis is a kshudra kushta. It occurs when there is imbalance of the vata and kapha dosha in our body.

Types Of Psoriasis

There are different types of psoriasis written as below

  • Plaque Psoriasis – It is the most common type of psoriasis. The typical lesion is a raised, well demarcated plaque and covered by silvery white scales.
  • Guttate Psoriasis – It is mostly found in children and adolescents. The lesions are small, droplet shaped, numerous and scaly.
  • Erythrodermic Psoriasis – It is a medical emergency but a very rare condition.
  • Pustular Psoriasis – It may be localised or generalised. Localised pustular psoriasis is common and mostly found on palms and soles. Generalised pustular psoriasis is uncommon and a life threatening condition.
  • Inverse Psoriasis – They appear as red patches in armpits, beneath the breast area. Rubbing between folds of skin can make it worse.
  • Nail Psoriasis – It causes pitting, whitening of the nails, yellow- reddish discoloration of the nails known as the oil drop or salmon spot, dryness, separation of the nails etc.

Causes Of Psoriasis

The exact cause of psoriasis is idiopathic or unknown, but it is an immune mediated disorder in which our immune system starts overacting and causing skin cells to multiply too quickly that is why skin becomes inflamed and scaly..other additional causes may be genetic, environmental, infection etc.

Some Exacerbating Factors Are

  • Sunlight may worsen psoriasis
  • Drugs like Beta blockers, NSAIDS etc
  • Trauma like surgical wounds and scratches
  • Stress

Signs And Symptoms Of Psoriasis

  • Cracked and dry skin
  • Red coloured patches with white or silvery scales typically on knee, elbow,.scalp, palms, soles.
  • Burning sensation
  • Itching and bleeding
  • Thick and pitted nails
  • Poor sleep

Complications of Psoriasis

  • Mental health issues like depression, anxiety
  • Cardiovascular issues-Stroke, heart attack
  • Psoriatic arthritis is the inflammation of the joints which cause swelling, pain, stiffness.

Diagnosis

  • History of patient- Doctor will ask when lesions first appeared, If they are itchy, painful and will ask whether people in your family have psoriasis etc.
  • Skin examination- location, size, shape and appearance of lesions.
  • Skin biopsy – It is generally not required.

Ayurvedic Point Of View

Psoriasis is characterised in Ek-Kushtha(psoriasis) that is a type of Kshudra Kushtha. There are three types of Doshas in our ayurveda – Vata, Pitta, Kapha. Psoriasis is the imbalance of the Vata and Kapha doshas. In Ek- Kushtha lesions appeared all over our body and have fish-like silvery scaly lesions. The lesion will be devoid of sweating which means the skin around the lesion will not sweat. According to ayurveda we can do shodhan chikitsa(elimination of the toxins from body) like Vaman karma(therapeutic emesis) with herbs like Madanfal (Randia dumetorum), Madhuka (Madhuca longifolia) etc. It will help in elimination of the kapha dosha from the body. Niruh Vasti (Therapeutic enema) with decoction of Daruharidra (Berberis aristata), Nimba (Azadirachta indica), Patola(Trichosanthes dioica) etc mixed with ghee or oil. It will help in elimination of the Vata dosha. There are some herbal drugs like Arogyavardhini vati, Kaishor guggulu, Ras manikya, Gandhak rasayan.
