Introduction

Posterior reversible encephalopathy syndrome (PRES), also known as reversible posterior leukoencephalopathy syndrome. The condition is usually present with rapid onset of symptoms. If the condition is promptly recognised then it may resolve in a week or else can become fatal. PRES or Posterior reversible encephalopathy syndrome usually coexist with kidney disease. Kidney disease which is the result of hypertension, vascular and autoimmune disease, exposure to immunosuppressive drugs and organ transplantation.

Causes of Posterior reversible encephalopathy syndrome (PRES)

The causes of PRES includes (In children)

• Renal insufficiency due to various etiologies
• Hematologic disease (Disease related to blood and its constituents) along with therapies.
• Cytostatic agents (the substance that stops cell growth) of corticosteroids for malignancy or immune suppression.

Causes of PRES in Adults

• Blood pressure fluctuations
• Preeclampsia (signs of high blood pressure in pregnancy)
• Renal failure
• Cytotoxic agents
• Autoimmune conditions

Sign and symptoms Posterior reversible encephalopathy syndrome (PRES)

The common symptoms are variable. Some may experience it and another may land in with serious difficulties. The symptoms are:

• Visual disturbances
• Blurred vision
• Homonymous hemianopsia (Person sees only one side either right or left) and it is due to problems in brain functioning.
• Cortical blindness (Loss of vision without any ophthalmological cause)
• Altered consciousness, which is present with mild confusion
• Agitation or coma
• Nausea along with vomiting
• Seizures

Diagnosing Posterior reversible encephalopathy syndrome (PRES)

There is no specific diagnosis of PRES. Generally, it is diagnosed when someone is having or has developed acute neurological symptoms like seizures, altered mental state, headache or visual disturbances.

In imaging tests there are certain common findings like vasogenic edema without inflammation, ischemia or neuronal damage. In later stages of PRES, evidence of ischemia, anoxic neuronal damage, laminar necrosis or older haemorrhage in the white matter and cortex.

Treatment of Posterior reversible encephalopathy syndrome (PRES)

The general treatment includes anti hypertensive drugs, withdrawal of offending agents or medicines along with anti convulsive therapy. In case of severe renal failure, temporary renal replacement therapy (hemodialysis, peritoneal dialysis) is also required.

The disease can be understood as the result of avarana. The avana in this case is done by other doshas over vata dosha. That means the avrit in this case is vata dosha. As the vata is avrit various problems are thus arising in this condition which hence hampers the functioning of the microchannels or srotas. Vata dosha, normality is responsible for all the movements going on in and inside the body but if the working or functioning of this dosha is hampered then obviously various types of symptoms can be observed in the body. The excess of vata makes it involuntary in or outside the body.
